Development of two advance directives and reflection paper. Each student will develop his/her own advance directive and identify an older family member (parent, grandparent, aunt, uncle) to develop his/her advance directive. The student will interview the older adult and document that person’s wishes on the advance directive. Students will engage in a discussion with their family member about advance care preferences and reasons for selecting/not selecting care options. After the two advance directives are completed and discussed, each student will write a 5 page (double-spaced) paper discussing their experience, learnings, and views on the usefulness of ADs/discussions.

If your parent already has an advance directive you can use that form, BUT you must discuss the details listed on the attached form. You can attach the form to the existing directive, if you like. The advance directives do not need to be signed nor do they need to be notarized for this assignment. Each student should submit the reflection paper along with the two completed directives
